1. Physician authorizes/orders remote patient monitoring 2. The specified RPM technology is deployed & activated in the patient's home 3. CHC monitors patient health data flow and adherence information 4. Upon detection of abnormal readings or non-compliance CHC alerts patient; can elevate to clinician when intervention required
